Frequently Asked Questions
What are conjugated equine estrogens?
Conjugated Equine Estrogens (CEE, brand name Premarin) are a mixture of estrogenic compounds — primarily estrone sulfate and equilin sulfate — derived from pregnant mare urine. CEE was the dominant menopausal estrogen for decades and is the formulation studied in the Women's Health Initiative (WHI).
Are conjugated estrogens still recommended?
CEE remains FDA-approved and effective, but most current guidelines favor bioidentical 17β-estradiol — particularly transdermal — for new starts. The shift reflects more favorable VTE/stroke profiles with transdermal estradiol and patient preference for bioidentical hormones.
CEE remains a reasonable option for women already stable on it or for women who tolerate it well.
What did the Women's Health Initiative actually show?
The WHI tested two regimens: CEE alone (in women without a uterus) and CEE + medroxyprogesterone acetate (MPA). The combined arm was stopped early for increased breast cancer, stroke, VTE, and coronary events. The estrogen-only arm showed no increase — and a possible decrease — in breast cancer.
Subsequent re-analyses by age have shown that women starting therapy within 10 years of menopause have a much more favorable risk-benefit profile than the original WHI population (mean age 63).
Is CEE FDA-approved?
Yes. CEE is FDA-approved for moderate-to-severe vasomotor symptoms, vulvovaginal atrophy, hypoestrogenism, and prevention of postmenopausal osteoporosis. Combination products with MPA or bazedoxifene are also approved.
Do I need a progestogen with CEE?
Yes, if you have a uterus. CEE alone in women with a uterus increases endometrial cancer risk. The Duavee product pairs CEE with bazedoxifene (a SERM) as an alternative to a traditional progestogen.
How long does CEE take to work?
Vasomotor symptom relief usually begins within 2–4 weeks and is maximal by 8–12 weeks. Vulvovaginal symptoms may take 8–12 weeks.
Can I get CEE without a prescription?
No. CEE is a prescription medication and requires a clinical evaluation, including a risk discussion, before initiation.
How do I find a doctor who prescribes CEE?
